Our geriatric-focused providers manage chronic conditions, medications, preventive needs, functional changes, and care transitions while coordinating closely with families, caregivers, and other healthcare professionals.

Home-Based Primary Care for Older Adults

Preferred Healthcare brings comprehensive primary care directly to older adults and medically complex patients who have difficulty leaving home. Our housecall program removes many of the transportation, mobility, and access barriers that can prevent vulnerable patients from receiving consistent medical care.

Our experiences providers evaluate the whole patient—not simply one diagnosis at a time. We manage chronic illnesses, medications, functional and cognitive changes, preventive needs, and transitions after hospitalization while coordinating closely with families, caregivers, home health agencies, specialists, pharmacies, and other members of the patient’s care team.


Our home-based primary care services include:

  • Comprehensive care visits in the patient’s home

  • Management of multiple chronic medical conditions

  • Medication review, reconciliation, and deprescribing when appropriate

  • Geriatric assessments for falls, frailty, memory loss, and functional decline

  • Post-hospital and transitional-care follow-up

What services do we provide?


We care for older adults and medically complex patients who are homebound or have significant difficulty traveling to a traditional medical office. This may include patients living in private residences, assisted living communities, and independent living communities.

Many of our patients have multiple chronic conditions, mobility limitations, dementia, frailty, recurrent falls, frequent hospitalizations, or a recent decline in their ability to manage medical appointments independently.

We also partner with families, senior-living communities, hospitals, rehabilitation providers, home health agencies, hospice organizations, and other referral partners seeking reliable ongoing primary care for vulnerable patients.

Who do we typically work with?

We provide access to care where the patient is.

Traditional office-based care can be difficult to access when a patient is frail, dependent on a caregiver, or unable to travel safely. A routine appointment may require transportation arrangements, time away from work for a family member, and a physically exhausting trip for the patient.

Home visits allow our providers to evaluate patients in the environment where they actually live. This provides valuable insight into medication use, nutrition, mobility, fall hazards, caregiver support, and the patient’s ability to follow the treatment plan.

Homebound older adults are among the most medically complex patients in the healthcare system, yet many have limited access to routine office-based care. A systematic review involving 46,154 homebound older adults found that eight of nine home-based primary care programs improved at least one major outcome, such as hospitalizations, emergency visits, hospital days, long-term care use, costs, or patient and caregiver experience. In the CMS Independence at Home Demonstration, participating practices generated more than $25 million in Medicare savings during the first performance year, averaging approximately $3,070 per beneficiary, while also meeting defined quality standards. A study of medically complex patients receiving home-based primary care found that hospital admissions, 30-day readmissions, and total inpatient days declined during the year after enrollment. (PubMed) Another Veterans Affairs program reported 43.7% fewer hospital admissions and 49.9% fewer hospital days after six months of enrollment, although these observational findings do not prove that every program will achieve the same results. Stephen Major, MD

Stephen Major, MD, is a Cincinnati-area pulmonologist and critical care physician in private practice with Long-Term Care Pulmonary Consultants and Preferred Healthcare.

He earned his medical degree from the University of Toledo College of Medicine and completed both his internal medicine residency and pulmonary and critical care fellowship at the University of Cincinnati. Board-certified in internal medicine, pulmonary disease, and critical care medicine, Dr. Major specializes in general pulmonology and the management of complex respiratory conditions.

George Knight, APRN

George Knight, CNP

George Knight, CNP, is a Cincinnati-based nurse practitioner and healthcare leader with more than 28 years of experience in geriatric care. A University of Cincinnati graduate, he has provided primary care in private homes, assisted living communities, and skilled nursing facilities throughout Greater Cincinnati since 1998.

His clinical expertise includes complex geriatric care, cognitive disorders, Alzheimer’s disease, and helping older adults maintain their health and independence.
