Pulmonary specialty care for nursing homes + plus primary care at home
Preferred Healthcare delivers specialized pulmonary and geriatric primary care directly to skilled nursing facilities and homebound patients — helping reduce hospital transfers, manage chronic conditions, and support independence where people live.
Pulmonary Services
Preferred Pulmonary Care delivers on-site specialty care for residents with COPD, ventilator or NIV dependence, tracheostomies, and OSA—including chronic disease management, respiratory PDPM classification, medication optimization, and response to respiratory changes.
We partner with facility teams to manage complicated respiratory conditions, accurately reflect PDPM classifications, and detect clinical deterioration early to avoid rehospitalizations.
Home-Based Primary Care
Preferred Healthcare provides primary care in the home for older adults and medically complex patients who have difficulty traveling to a medical office.
Our geriatric-focused providers manage chronic conditions, medications, and care transitions — coordinating with families and caregivers to help patients stay safer and more independent where they live.
Comprehensive care, delivered wherever needed.
Preferred Healthcare is driven by a commitment to bring high-quality care directly to patients. We know complex patients cannot always make it to external appointment, so we bring the care to them. Through our nursing-home pulmonary program and home-based primary care services, we work to improve access, recognize changes earlier, reduce fragmented care, and help medically complex patients achieve better health and quality of life.
From complex respiratory conditions like COPD, ventilator dependence, and chronic respiratory failure to ongoing primary care in the home, Preferred Healthcare brings specialized expertise to the bedside—wherever that bedside happens to be.
